Diabetes Standards of Care

Diabetes can be as complex for practices as for patients because of data inaccuracies, health illiteracy, non-adherence, and strained staffing models. Glucose Guards helps you overcome these barriers to better care while also generating significant revenue for your practice.

Remote Patient Monitoring (RPM)

Remote Patient Monitoring (RPM) uses digital technologies to monitor and transmit health data in real-time, leading to earlier and more meaningful clinical intervention. RPM can improved patient outcomes and lead to higher satisfaction while generating significant revenue for practices.

Glucose Guards empowers your practice to build and maintain a successful RPM Program by providing everything from software, staffing, and hardware all the way through billing support. This allows you to focus on what matters most – patient care.

Medical Nutrition Therapy (MNT)

Medical Nutrition Therapy (MNT) is a critical standard of care identified by the Centers for Disease Control & Prevention and American Diabetes Association. Diabetes Self-Management Training (DSMT) and MNT are complimentary services which should be prescribed to every patient with diabetes to maximize their cost-effective benefits.

MNT involves 1-on-1 or group sessions with a registered dietician to provide personalized dietary recommendations to patients with consideration to their medications, comorbidities, and cultural preferences.

Medicare reimburses 3 hours of MNT per year in full at no cost to the patient. Otherwise coverage varies by provider.

Diabetes Self-Management Training (DSMT)

Diabetes Self-Management Training (DSMT), also known as Diabetes Self Management Education / Support (DSME/S), is a critical standard of care identified by the Centers for Disease Control & Prevention and the American Diabetes Association.

DSMT involves targeted education that focuses on positive self-care behaviors for people with diabetes. The topics include the causes of diabetes, testing recommendations, target ranges, medication side effects, complications, healthy eating, physical activity, and problem solving/coping mechanisms.

Medicare reimburses 10 hours of DSMT the first year it’s prescribed. Subsequent years include 2 hours of continuing education. DSMT has a co-pay which varies by carrier but is around 20%, or $15.
